How do I create a survey page?
A survey page allows you to gauge public opinion on a specific initiative, get feedback on policies, or enables your supporters to share important interests with your campaign! To create a survey page select New Page and Survey Page from the available options in the dropdown.
Next, you'll want to customize the form itself, including providing a Name and Template (required), Source Code and Member Code (optional), Survey Form Headline (required), and Survey Form Supporting Text
You'll be able to add Form Components, the fields that a supporter will include with their submission. In this section, you can add default or custom fields (learn more about how to add custom fields here) like text input, text area, dropdowns, checkboxes, and radio.
Lastly, by default you can redirect your supporters to another Action Page, Numero Contribution Form, ActBlue Form, or Other, like your website! If you want to opt for your supporters to see a Thank You page, instead of redirecting them, please toggle the Enable Customer Redirect field to No. Be sure to select Save Page in the top right corner to save or update changes that were made!
